I ____________________________________ (“Participant”) desire to participate in training, running and/or walking with “Team FDC”. In consideration of my participation with “Team FDC”, I intend to be legally bound, to hereby, for myself, my heirs and executors, waive, release and forever discharge any and all rights and claims for damages which I may have hereafter accrue to me against Team FDC, LLC., coaches, its officers, agents, representatives, successors and sponsors of any and all damages, injuries (including death), lawsuits, expenses (including attorneys’ fees) and any other liability sustained by me, my equipment or personal property which may occur directly or indirectly from my participation in training or races, whether by negligence, breach of contract or in any manner whatsoever. I attest that I am in proper physical condition to participate with “Team FDC”. I hereby grant permission to "Team FDC" and its sponsors to use all information submitted on my Registration Form and any photograph, videotape, motion picture, recording and any other record of my attendance at or participation in the Training Program including my likeness and name for any lawful purpose related to publicity and the needs of "Team FDC" and its sponsors.
You must be over 18 years of age OR the parent/legal guardian of a minor under 18 years of age OR the legal guardian of an incapacitated and/or mentally challenged person in order to agree to the text above.
